Registering a New Patient
This flow outlines the steps for a physician to register a new patient within the MedTrack platform.
- Login: Access the MedTrack Physician Dashboard at https://physician.medtrack.io and log in using your credentials.
- Navigate to Registration:
- Go to the Patients section from the main navigation (https://physician.medtrack.io/patients).
- Click the Register button located in the top-right corner of the page. This will open the patient registration form.
- Alternatively, you can access registration via the In Patients section (https://physician.medtrack.io/in-patients) by clicking its Register button.
- Complete Patient Information (on the https://physician.medtrack.io/registration page):
- Personal Details: Enter the patient's First Name, Last Name, Date of Birth, and select their Gender.
- Identification: Indicate if the patient possesses a Ghana Card. If yes, enter the Ghana Card ID.
- Contact Details:
- Enter the patient's Phone Number. The system will automatically check if a patient with this number already exists.
- Enter the patient's Email address (optional).
- If the patient does not have a phone number, check the box labeled "Patient does not have a phone number". You will then be required to enter details for a Dependant Contact (Name, Phone Number, Relationship to Patient).
- Location: Select the patient's Region and enter their City/Town and Address/Digital Address.
- Nationality: If your facility requires it (e.g., screening facilities), select the patient's Nationality.
- Complete Emergency Contact Information (Optional) (on the https://physician.medtrack.io/registration page):
- Enter the Name, Phone Number, and Relationship of the person to contact in case of an emergency.
- Submit Registration (on the https://physician.medtrack.io/registration page):
- Review all entered details for accuracy.
- Click the Register Patient button at the bottom of the form.
- Confirmation/Error Handling:
- Success: Upon successful submission, the system creates the patient record, a confirmation message will appear, and the patient will be added to the facility's records. You may be redirected to the main patient list.
- Error: If any required information is missing or invalid (e.g., duplicate phone number, invalid Ghana Card format), error messages will appear next to the corresponding fields. Correct the highlighted errors and click Register Patient again.
